How to submit a claim

Firstly, you will need to download and complete the medical claim form which you can access by clicking here.

Attach your completed medical claim form in an email alongside a Word document/note of a couple of paragraphs detailing exactly what happened during the medical emergency incident, the medical center you had to attend, and every medical appointment you had. 

Please attach all documents and receipts you received from the medical center during your visit as well. Once you have all of these items, send your claim to mail@oncallinternational.com and await a response.
